Your preceptor’s details and your school’s preceptor form

The short answer

Your preceptor’s details sit in the Matched preceptor section under your placement order, set out as the lines a school’s preceptor form asks for: name, credentials, contact, education, licenses, NPI, facility and site administrator. From the Matched step you see their first name and facility, more of the profile opens as your placement moves forward, and at the Application step their full details and documents show, ready for your school’s preceptor form.

Where your preceptor’s details live

Open the Orders tab and find your placement order. At the bottom of the order sits the Matched preceptor section, with your preceptor’s details in two columns and their documents underneath. It is open by default, and the bar at its top folds it away and opens it again whenever you like.

The bar also tells you, in one short line, what the section holds right now, and a status beside it reads Not matched yet or Preceptor matched. Each placement order has its own section, so if you have more than one order, each preceptor’s details stay with the placement they belong to.

What you see at each step

The section fills in as your placement moves forward through its eight steps. Here is what to expect along the way:

StepWhat the Matched preceptor section shows
Booked and ReservedIt is blank, with the line Blank until a preceptor is matched to this order, while the search for your preceptor runs.
MatchedYour preceptor’s first name and facility. More of the profile opens as the placement moves forward.
Confirmed and MeetingMore of the profile, with some lines reading Shown at Application. The bar reads Their full name, contact and documents show at the Application step.
Application onwardTheir full details and documents, ready for your school’s preceptor form. The email and phone lines may read Through your coordinator.

Every line in the section

The lines follow the order most schools use on their preceptor forms, and each license number sits beside its expiration date. A line that isn’t filled in yet shows a dash.

LineWhat it is

Your preceptor’s documents

Under the details, the Documents list holds the files your preceptor has on file: their resume, their license and any other document. Each file shows its name, its type, the date it was added and its size, with a Download button beside it.

Before a match, the list explains what will appear there. Before the Application step, it reads The preceptor’s documents show at the Application step, and from Application on, the files are ready to download. If your school asks for a document that isn’t listed, reply to any message from your placement manager and say which one you need.

Filling in your school’s preceptor form

Most schools ask for a preceptor form, sometimes called a preceptor agreement or a clinical site request, as part of your clinical application. The Matched preceptor section is laid out so you can fill it in line by line:

  1. 1Wait for the Application stepYour order reaches Application when your clinical application goes to your school, and your preceptor’s full details show then.
  2. 2Open your school’s formHave it open beside your order, on paper, as a document or in your school portal.
  3. 3Copy each line acrossWork down the section from Preceptor name to Administrator phone, and match each line to the same question on your school’s form.
  4. 4Download the documents your school asks forMost forms ask for a resume or CV and a copy of the license. Download them from the Documents list and attach them.
  5. 5Check the details once moreCompare license numbers, expiration dates and the NPI digit by digit, because a single wrong digit can hold up an approval.
  6. 6Submit it the way your school asksFollow your school’s own steps for sending the form, and reply to your placement manager if anything is unclear.

What Shown at Application and Through your coordinator mean

Shown at Application marks a line that is held until your order reaches the Application step. Nothing is missing: the line fills in on its own when your order gets there, at the moment your school’s preceptor form needs it.

Through your coordinator can appear on your preceptor’s email and phone lines. It means contact with your preceptor goes through the placement team, so reach out by replying to any message from your placement manager, and they connect you. Every other line of the section still shows as usual.

Before your preceptor is matched

While your order is at Booked or Reserved, the section stays blank and the status reads Not matched yet. This is a good time to get everything else ready: add your school portal login in Settings, and send your affiliation forms, school approvals, license and health records to your Dropbox.

When a preceptor is matched, the section opens with their first name and facility, and the Overview shows your preceptor match as done.

Questions students ask

When do I see my preceptor’s full details?

At the Application step. Before that, you see their first name and facility from the Matched step, and more of the profile opens as your placement moves forward.

Why does a line say Shown at Application?

That line is held until your order reaches the Application step, and it fills in on its own when it does.

What does Through your coordinator mean?

Contact with your preceptor goes through the placement team. Reply to any message from your placement manager and they connect you.

Where do I download my preceptor’s license and resume?

In the Documents list under the Matched preceptor section of your order, once your order reaches the Application step.

A line on my order shows a dash. What does that mean?

That line isn’t filled in yet. If your school’s form needs it, reply to any message from your placement manager.

My school’s form asks for something not listed. What do I do?

Reply to any message from your placement manager, name the line your school asks for, and mention your order number.
